Step-by-Step Guide

5. Confirm your appointment – you’ll receive an email/text confirmation.

ICBC or WorkSafeBC Cases

For car crash or workplace injuries:

• Complete the intake form during booking.

• Provide your claim number, date of injury, and any prior medical history.

• Our MOA team will review and confirm your appointment.

U.S. or Private Pay Patients

• Complete the U.S. Patient Intake Form.

• Provide ID/passport & health history.

• Payment is required before appointment.

• A detailed invoice will be provided for insurance claims.
