There are several reasons why patients may not adhere to their prescribed medication regimen. Some patients may forget to take their medications, while others may not understand how to take them properly. For some patients, physical or cognitive barriers may make it difficult to manage their medications effectively.

At CityMed Pharmacy, we recognize the importance of medication management and have developed a home monitoring program to address this issue. Our program provides home visits by well-trained Licensed Practical Nurses (LPNs) to manage and monitor chronic conditions. We also provide training on how to use medications and specialized devices such as blood glucose meters, nebulizers, compression stockings, Aero chambers, and more.

Our daily home visits include administration of medications, applications of topical patches, instilling eye/ear drops, helping with inhalers, injections, blood glucose and blood pressure monitoring as per care plan, administering and/or training with Insulins as per Physician’s order. Our program is designed to provide comprehensive care and support to patients in the comfort of their own homes.

How It Works

Our home monitoring program is referral-based. Patients must be referred by their doctor, another healthcare professional such as a nurse, a case manager, or a health unit. The referral form can be faxed at +1 (604) 385-1491 to one of our pharmacies, or your professional can call the referral line at  +1 (604) 385-1490.

Enroll Today

Enrolling in our home monitoring program is simple. There is no charge to the patient except for the cost of the patient’s prescription medication. Certain criteria may apply.
